To aid in reducing hospital readmissions, Kennedy Park Nursing & Rehab Center has begun a new Chronic Heart Failure (CHF) Rehab Program. This evidence-based clinical program was developed for Physical and Occupational Therapists to help enhance treatment outcomes for CHF patients. The program utilizes neuromuscular re-education and other types of treatments for residents who have reduced physical activity levels and tolerance.
